Health, Genetic and Social History
The social health history will give your child and the adoptive family information about the genetic and health history of you and the birthfather. This will help them know what health conditions run in the biological family such diabetics, heart conditions, etc. This information is needed to help adoptive families know what health concerns to watch out for. We will need it to help match you with a family.
This will take a minute to complete. Do not worry if there is information you don’t know, just complete it the best you can. If the birthfather is available, he can help, but otherwise provide the information you know.

If you have access to a printer, you can download both of these documents here. You'll print two documents: one for the birth mother and one for the birth father.
Step 3: Complete and mail them to us
Complete the form for the birth mother and complete the form for the birth father. and return them to our offices by mail at the address below.

If you don't have access to a printer, please send an email to firstname.lastname@example.org and request to have these documents mailed to you.
Step 3: Complete and mail the forms back to us.
We'll send them to you with a postage-paid, return envelope. All you have to do is fill them out and send them back.
Even if you don’t know all the answers, any information you feel you can give us will one day help your baby to understand their medical history.
